What this actually is

Where the need has crossed from support into treatment. Every clinical visit is recorded against the beneficiary’s chart — vitals, medication administered, refused or held, and the nurse’s own notes — so a treating doctor can read what actually happened between appointments.

What is included

  • Vitals recorded to the chart: blood pressure, pulse, SpO₂, temperature, glucose
  • Medication administration logged with outcome, including refusals
  • Wound dressing, catheter and stoma care, injections
  • Post-operative and palliative support
  • Escalation to the coordinator when a reading leaves its safe range

What is not

  • Diagnosis and prescription — the nurse follows a doctor’s written orders
  • Controlled drugs not supplied with a valid prescription
  • Emergency resuscitation beyond first response and ambulance activation
